Why IBS Patients Develop Anemia: 4 Real Pathways

Let’s be specific about how a “functional” gut disorder that supposedly causes no structural damage can still leave you iron-depleted and anemic.

1. Restrictive Diets That Cut Out Iron-Rich Foods

This is the most common and most overlooked pathway. Up to 70% of IBS patients report avoiding specific foods to control symptoms. The low-FODMAP diet — the gold standard dietary intervention for IBS — eliminates or limits several iron-rich food groups including legumes, certain vegetables, and fortified grains.

What Labs Should IBS Patients Get?

If you have IBS and you’re fatigued, don’t accept “that’s just part of IBS” as an answer. Request the following workup:

  • Complete blood count (CBC) — checks hemoglobin, hematocrit, and MCV (red blood cell size)
  • Ferritin — the most sensitive early marker of iron depletion; levels below 30 ng/mL indicate deficiency even if hemoglobin is still “normal”
  • Serum iron, TIBC, and transferrin saturation — helps distinguish iron deficiency from anemia of chronic disease

  • Fatigue that isn’t explained by poor sleep or stress alone
  • New onset of heavy or prolonged menstrual periods combined with GI symptoms
  • Pale skin, brittle nails, cravings for ice or non-food items (pica — a classic sign of severe iron deficiency)
